FAQ

Assisted Living Services are available in both the Colonial Terrace and the Care Center Apartments settings.  The best fit resident for our Assisted Living is a person over the age of 65 or younger with service needs.  This resident would be independent in their dressing, eating and transferring but may need assistance with meal preparation, medication administration, bathing and household chores.  Residents that wander would not be safe in this setting.

The Clarissa Manor is our independent living setting.  The best fit resident is a person over the age of 55 or younger with disabilities/special needs.  Residents of the Clarissa Manor need to be independent in all of their daily activities, and no assisted living or homecare services are provided by the Care Center.  Rental assistance is available through Todd County HRA for low income residents.

Rehabilitation Services are available to people with Rehab needs of any age.  We offer Physical, Occupational and Speech therapies on an outpatient basis.  Therapy services are provided by Bigstone Therapies Inc, who partner with the Care Center for Therapy needs.  We can provide recovery therapy after surgery and illness in a clinic setting.  Examples include:  Sports injury, joint repair/replacement (including shoulders, knees, hips and ankles), balance and strengthening, wound care etc.

We accept all major medical plans including Medicare, Medicaid and MnSure.  We have contracts with Blue Cross, Medica, Ucare, and Humana (for therapy only)

The Care Center does not accept Humana plans at this time for Nursing home stays.

Nursing home:  Resident will be given a list of essential items, but should plan to bring clothing, personal toiletries and a razor.  Each nursing home room is furnished with a bed, nightstand, chest of drawers, Armoire, chair, and bedside table.  Residents are encouraged to bring items from home to personalize their space including pictures, paintings, small furniture pieces and other decorations.  Many residents prefer to have their favorite recliner in their room.  Satellite Television is available at no cost.  Residents will need to bring their own cable ready television.

Assisted Living:  All apartments are unfurnished.  Arrangements can be made with the Care Center to provide necessary furniture/supplies in urgent situations.  Wall decorations and furnishings are encouraged to make the space feel like home.

Current Visitor Policy: 
Independent living- no restrictions on visitors. 
Nursing Home and Assisted Living:  Visitors are welcome to visit. 

Visitors must be screened prior to entry.  Screening involves temperature, current symptoms (cough, sore throat, fever), recent exposures, loss of taste or smell, headache, muscle pain, chills.  Visitors with recent gastrointestinal symptoms (nausea, vomiting, diarrhea, abdominal pain not related to a known diagnosis) are asked to leave a return two days after symptoms resolve.

Visitors will be limited to in-room or in- communal space visits with their resident.  Visitors are asked not to visit multiple residents or with staff for contact tracing purposes if we have an outbreak.

Visitations will be suspended if we have a positive Covid test in residents or working staff until outbreak testing can be completed, isolation precautions are implemented, and surveillance testing is completed to ensure the outbreak has been contained.  Resident representatives will be contacted via instant alert regarding outbreak status and any changes in visitation.

Pet visitation.  Pet visitation is encouraged.  Pets need to be under control while in the nursing home.

Compassionate visits for End of Life and other exceptional reasons will be honored as best as we can make possible regardless of outbreak status.

Staff are required to wear appropriate mask and protective eyewear while performing work functions on and off premise and screen prior to each shift.  Visitors are required to wear face coverings and are required to screen in prior to each visit.

Testing: 

Surveillance testing only consists of unvaccinated staff (CTCCC staff, volunteers, clergy, therapy contractors, and providers).  Surveillance testing frequency is dependent on Todd County positivity rate.  <5% is monthly surveillance testing, 5-<10% is weekly testing and >10% is twice weekly testing.

Outbreak testing is testing of all staff, volunteers, clergy, therapy contractors, providers AND residents regardless of vaccination status.  Outbreak testing is performed when a resident or working staff members tests positive for a covid infection.  Outbreak testing will continue weekly (or less) until outbreak status can be cancelled.

Activities and meal services are socially distanced, and residents are asked to wear a mask when outside their room.

Our employees are made up of amazing caregivers of whom which most reside within 20 miles of the Care Center.  We employ nurses, nurse aides, activity aides, dietary aides, cooks, environmental service and maintenance personnel, and other clerical staff.

We provide a host of activities for the residents including bingo, happy hour, bowling, cards, nail painting, Men’s hour, Sensational Tuesdays, Reminiscence, miscellaneous group activities/games and one to one service.
